First Communion Registration

Complete this form to register your child for First Communion classes at Mar Abraham (Scottsdale) or Holy Cross (Gilbert). Please read the policies below carefully before submitting.

Which Church? *

Your child must be in 3rd grade or higher this school year.

Mar Abraham Saturdays, 10–12PM Starts September 12
Holy Cross Sundays, 3–4:45PM Starts September 13

First Communion Day and photo/video/gown pricing are announced closer to the semester.

Policies

Attendance Policy: I understand that if my child or children have more than 3 unexcused absences they will not receive their First Holy Communion.

Classroom Rules: Disruptive students with 3 or more warnings will not be allowed to receive their First Communion. Parents are not to enter any of the classrooms at any time.

Drop Off & Pick Up Policy: Child(ren) must be dropped off at Mar Abraham by 9:45AM and picked up no earlier than 12PM. Child(ren) must be dropped off at Holy Cross by 2:50PM and attend Mass with their parents.

Parental Consent: I understand that reasonable precautions will be taken to safeguard the health and wellbeing of participants in this Catechism Class and that I will be notified as soon as possible in the event of an emergency. In case of sickness or an accident, I authorize the Catechism Team or associated volunteers to obtain medical care from a licensed physician, hospital, or medical clinic for my child if I or the other legal guardian(s) cannot be reached. I release Mar Abraham Chaldean Catholic Church & Holy Cross Chaldean Catholic Mission, the Chaldean Catholic Diocese of St. Peter the Apostle, and their officers, directors, board members, and volunteers from all claims arising during my child's attendance of the Catechism Class. Unless other written instruction is submitted, I consent to my child's image being recorded (photo or video) and used during Catechism Classes and the First Communion ceremony online or in print.

Registration Fee: $100 (Mar Abraham & Holy Cross). Payment will be collected online once our payment system is connected — for now, submitting sends your registration to the parish office.
